I confirm that I am either the adult swimmer providing my own consent or the parent/legal guardian providing consent on behalf of a child. I consent to the collection and processing of the personal data provided in this form for the purposes of class registration, safety, and communication, as described in the Privacy Notice.*

I consent to Gold Fins Swimming School collecting and processing medical information provided for the purpose of ensuring the safety and well-being of the swimmer during lessons.*

*Optional* I give permission for Gold Fins Swimming School to take and use photographs or videos of me / my child for promotional purposes, including on social media, the school’s website, and marketing materials.
